This 6-part traditional self mailer carbon-interleaved set includes Employer Copy A (federal, red scannable), 1 (state) and D (file) on top of the employee copies B (federal), 2 (state) and C (file), which are pre-inserted into an envelope. When you use your typewriter or pin-fed printer, the mailing address will also be printed on the envelope. You just seal and mail! Forms are 8½” wide (format changed in 2001). Order the quantity equal to the number of employees for which you need to file. Mail employer copies in a batch to the SSA. Employee copies are ready to mail.
