This 6-part carbonless form includes a set of forms for employers, including copies A (federal, red scannable) 1 (state) and D (file). A second set of forms for employees includes copies B (federal), 2 (state) and C (file). Your typewriter or pin-fed printed will print the employer and employee forms in 2 different passes. This ensures the legibility of all information on all parts. Forms are 8 ½” wide (format changed in 2001). Order the quantity equal to the number of employees for which you need to file.
